Nutrition Care process

Registered Dietitian Nutritionists utilize the "Nutrition Care Process" to ensure clients receive high quality nutrition care. This process allows a framework to individualize care, take into account specific needs and values of clients and set the client up to meet goals. Erin will utilize this process during each meeting.

The Nutrition Care Process includes 4 steps:

1. Nutrition assessment

2. Diagnosis, or naming and discussing the problems at hand

3. Intervention, or addressing the root cause of each problem, aimed to alleviate signs and symptoms of the diagnosis and achieve goals

4. Monitoring/Evaluation to determine if the client has achieved or is making progress towards goals. Adjust goals and plans as needed.
